Medical Forms

Medical Forms (parts A&B) and a photocopy of insurance cards (front and back) must be provided to your den leader each year. Medical forms are required for all campout attendees including Scouts, siblings, parents, and grandparents. 

An immunization exemption request form is available for those requesting an exemption on religious, philosophical, or medical grounds.
